Don't miss this opportunity to meet and share information with other vitreoretinal specialists in an open, intellectually demanding, collaborative environment.  Participants are invited to bring case presentations on vitreoretinal surgery, retinal diseases, medical retinal treatments and other related topics. This program is designed for vitreoretinal specialists and is highly regarded by previous attendees.

If you are planning to present a case as part of your participation, Emory CME will need a signed financial disclosure form returned by April 1, 2015. In alignment with ACCME Standards and Criteria, a financial disclosure form MUST be received by this date in order to present before the attendees. No exceptions. Accreditation

The Emory University School of Medicine designates this live activity for a maximum of 10.5 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

The Emory University School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Tuition

The early registration fee for this meeting is $400 until February 28, 2015. After February 28, 2015, the registration fee is $500. The tuition fee includes the meeting space, materials, continental breakfasts, coffee breaks, lunch, social event and CME credit will be provided. Please register by February 28, 2015.

Faculty Social (Reception and Dinner): A social function with dinner for attendees will be held at 7:00pm, Friday, April 24, 2015. Please indicate on the registration form whether or not you will attend. If attending, please note how many guests including you will attend. The location for the dinner social will be announced.

Please check back to register.  Registration is required. Please register by April 1, 2015. AMEX, Discover, Visa and Mastercard are accepted. First time registrants will need to set up a password at the CME registration site. This account will also allow access to the CME credit certificate after attending the seminar.

CME Cancellation Policy

A maximum enrollment has been established, so register early. Written cancellations received prior to April 1, 2015 will be refunded less a $75 administrative fee. No refunds will be given after this date. Emory University reserves the right to cancel the program and return all fees. The liability of Emory University School of Medicine is limited to the course fee. Emory will not be responsible for any losses incurred by registrants, including, but not limited to, airline cancellation charges or hotel deposits.

A block of rooms is available at the Emory Conference Center Hotel at a special discounted rate of $139 per night, single or double occupancy (plus tax). Reserve accommodations and receive the discounted rate or call (404) 712-6565 or  (800) 933-6679. Reservations should be made as soon as possible. Reservations are based upon space and rate availability. Make your reservation early as rooms may sell out quickly.
