Electronic access to Emory Eye Center requires access to the Emory Eye Center Patient Portal and the Emory Healthcare BLUE Patient Portal. Do not use portal messaging for urgent or emergency health issues.
Messages are reviewed 8 a.m.-5 p.m., Monday-Friday (except for holidays). Every effort is made for a same-day reply during these business hours, but please allow up to 24 business hours. Send messages only regarding yourself.

After enrollment in the patient portals, patients will have online access as follows:

• View and update contact and demographic information, pharmacy, medications, allergies, and past medical history
• View selected provider visit notes, patient-specific eye health education information and imaging test results
Emory Healthcare Blue Patient Portal
• Send secure messages to your Emory Eye Center provider and care team;
• Request prescription renewals
• View labs and radiology
• Request prescription renewals

IMPORTANT: Standard email and Facebook private messages are not secure and should never be used for online transmission of medical or other confidential information.

If this will be your first visit to Emory Eye Center, please read the following documents and download and print the Ophthalmology Patient Questionnaire (#1 below) to complete and bring with you to your first appointment. Notice: Due to patient confidentiality laws, we cannot accept the questionnaire electronically.

Please fax records and labs (if applicable) to the fax number in the referral form (below). If an urgent appointment is being requested, please mark notes urgent, fax notes and call 404-778-2020. The referring provider’s office will be contacted after notes are reviewed by a physician. Please have referring office/parent/patient call (404) 778-2020 to register patient and schedule appointment.
